Job summary

Responsibilities

• Assess needs and identify problems; devise care plans with patients and carers.
• Plan, implement and evaluate interventions to address identified needs and risks.
• Maintain safe custody, transportation and administration of medicines.
• Participate in team duty rotas and contribute to service development within governance frameworks.
