Job summary

Qualifications

• Health or social care professional with expertise of working within Mental Health services.

Responsibilities

• Lead discharge planning to enable safe and timely discharge from admission to bed release.
• Work within a multidisciplinary team to ensure continuity of care from hospital to community.
• Identify, assess, set goals, plan, implement, coordinate and evaluate discharge plans.
• Prepare agreed assessment reports and treatment plans in the accepted format.
• Make applications for funding discharge packages based on MDT recommendations.
