Job summary

Cornwall Partnership NHS Foundation Trust in Redruth invites applications for a Band 6 Discharge Liaison Mental Health Practitioner to support safe and timely discharge across Longreach Town, Redruth. The post is 37.5 hours per week and permanent, with a mix of full- and part-time opportunities.

The role involves multidisciplinary discharge planning, working within the Patient Flow team to reduce delayed transfers of care and coordinate care from hospital to the community.

Qualifications

• Registered health professional with NMC or HCPC registration.
• Experience delivering care/support to people with mental health issues.
• Experience leading discharge planning for people with complex needs.

Responsibilities

• Ensure the dignity and respect of service users.
• Signpost service users to access benefits and welfare resources.
• Follow planned interventions and monitor service users' progress.
• Prepare agreed assessment reports/treatment plans in the accepted format.
• Make applications for funding discharge packages based on MDT recommendations.
• Maintain adequate records and participate in electronic recording.
