Healthy communities are underpinned by better mental and physical health. Not all aspects of community life are accessible to all adults—social isolation, unemployment, financial and housing issues, socioeconomic deprivation, and domestic problems can negatively impact health. Mental Health Community Connectors in Rotherham play a key role in engaging and supporting individuals to overcome barriers, enabling access to community support and services to improve health outcomes.
These connectors are part of the transformation of community mental health care in Rotherham, aiding the NHS in moving towards integrated community mental health services across primary and secondary care, in collaboration with Rotherham Local Authority and the Voluntary and Community Sector (VCS). This integration aims to bring services closer, making them more responsive, accessible, and effective in achieving better health outcomes.
As a Mental Health Community Connector, you will be part of a multidisciplinary team within the Rotherham Primary Care Integrated Mental Health pathway, working alongside clinical colleagues to deliver holistic care by:
Building strong links with local VCS organizations and neighborhood groups, supporting the development of new groups and services where needed.
Applicants should have experience working in the community sector or NHS, paid or voluntary, and an understanding of the wider determinants of health, including social, economic, and environmental factors. As an effective team player, you should be able to motivate and influence others through a person-centered, strengths-based approach, demonstrating personal drive and resilience.
