Job description

Healthy communities are underpinned by better mental and physical health. However, not all aspects of community life are available to all adults - social isolation, unemployment, financial and housing problems, socioeconomic deprivation and domestic issues can all have a negative impact on a person's mental and physical health. Mental Health Community Connectors in Rotherham play a key role in engaging and supporting people to overcome the barriers that they may experience - enabling them to access community support and services to improve their health outcomes.

Mental Health Community Connectors are part of the transformation of community mental health care in Rotherham - helping the NHS to move towards integrated community mental health services across primary and secondary care in collaboration with Rotherham Local Authority and the Voluntary and Community Sector (VCS). This means services are brought closer together, more responsive, easier to access and achieve longer lasting and better health outcomes for patients.

As a Mental Health Community Connector you will be part of a multi-disciplinary team within the Rotherham Primary Care Integrated Mental Health pathway and work alongside clinical colleagues to help deliver a holistic care approach for patients, by:

  • Helping people to access support from local VCS organisations and services to meet their mental health and wellbeing needs.
  • Co-producing personalised support plans with individuals, their families and carers to take control of their wellbeing, increasing capability and motivation to improve their mental and physical health.
  • Drawing on and increasing the strengths and capacities of local communities, enabling local VCS organisations and community groups to provide opportunities for people to develop friendships, a sense of belonging, and build knowledge, skills and confidence.

Forging strong links with existing local (VCS) organisations and neighbourhood level groups you will also be involved in supporting local partners and commissioners to develop new groups and services where needed.

As well as having experience of working in the community sector or the NHS in a paid or voluntary role, you will have an understanding of the wider determinants of health including social, economic and environmental factors. An effective team player, you will also have the ability to motivate and influence others through a person-centred, strengths-based approach and possess personal drive and resilience.
