North London has a vacancy for a Mental Health Nurse (Care Co-ordinator). The core of this role is to support the delivery of care, currently in the form of New Models of Care.
Our North London NHS Foundation Trust operates at London Prison (HMP Pentonville). The ideal candidate will need to ensure they operate in line with the New Models of Care by providing high standards of care to individuals within our prison settings who are presenting with mental health problems.
The post holder will work with staff across healthcare and the wider prison establishment, providing specialist advice, particularly to GPs, in the assessment and treatment of patients on the prison residential wings.
The successful candidate will need to adapt to and embrace a recovery and enablement model to seamlessly realign prisoners with mental health issues with their local community-based mental health services upon release from prison.
The Mental Health Nurse (Care Co-ordinator) will be a qualified RMN with experience in Mental Health, knowledge, and experience of forensic mental health service provision and offender care.

Main duties of the job
- To work as a member of the Mental Health Service at HMP Pentonville.
- To work across service functions including early days in custody, planned care, crisis care, and release and transfer.
- To implement sound clinical skills in mental health practice, including CPA care coordination, group work, risk assessment and management, and crisis intervention.
- To be responsible for managing your own caseload as directed by the Mental Health Hub Manager.
- To provide skilled assessments of patients' emotional and psychological needs based on understanding, interpretation, and integration of complex data from the patient and other sources.
- To provide psychosocial interventions for patients with moderate to complex mental health difficulties.
- To facilitate group work jointly with multidisciplinary colleagues, including education staff.
- To complete clinical risk assessments at initial and subsequent assessments and accurately record and report key risks.
- To develop care plans collaboratively with the patient, carer, and other relevant persons or agencies.

Job responsibilities

- To act as CPA coordinator, initiating and reviewing care plans under enhanced CPA in consultation with the MDT.
- To serve as lead professional for caseloads not under CPA.

Person Specification
Qualifications
- Dip/BSc (Hons) in Mental Health Nursing, Social Work, or Occupational Therapy.
- Evidence of post-registration education and training or equivalent teaching and assessing qualification.
- MSc / MA (Hons) is desirable.
Experience
- Post-registration experience as a Band 5 Mental Health Nurse/OT or Social Worker, preferably within a forensic setting.
- Experience working with neurodevelopmental disorders.
- Ability to undertake risk assessments using standardised or semi-structured methods.
Skills
- Ability to co-ordinate within CPA, network, collaborate, and liaise with other agencies.
- Competence in handling challenging and complex situations and engaging effectively with patients and involved parties.
- Ability to work independently and autonomously.
- Excellent oral and written communication skills.
- Knowledge of the Mental Health Act 1983 Part II and Part III (as amended by the Mental Health Act 2007).
