BCUHB North Wales is seeking a Band 6 Mental Health Nurse to join the Memory Assessment Service across Wrexham and Flintshire. You will contribute to initial cognitive assessments, diagnostic discussions and early treatment plans within a busy MDT.

The role involves clinic and community visits, autonomous decision-making in rural areas, and leading aspects of MAS in the manager's absence. Welsh language skills are desirable and Welsh or English speakers are welcome.

Responsabilités

• Carry out in-depth first-line cognitive assessments and present findings to the MDT.
• Collect and collate information to inform diagnostic discussions.
• Provide support during early days of diagnosis and initial treatment planning.
• Work across clinic and community settings in a large rural area.
