The post will divide time between clinic and community settings across a large rural area in North Wales and requires autonomous working, complex decision-making, and MDT collaboration.

Responsibilities

• Lead an area of the Memory Assessment Service in the absence of the manager.
• Divide time between a clinic and community care setting across a large rural area with limited transport links.
• Conduct patient home visits and make complex decisions autonomously in isolated areas.
• Participate as a full member of the multi-disciplinary team and manage a caseload including initial assessments, diagnosis and treatment planning.
