Job summary

Kent Community Health NHS Foundation Trust seeks an Medicines Optimisation Assistant to support training, audits, and medicines safety initiatives within the Medicines Optimisation Team.

You will work with GP practices and PCNs to promote safe, cost-efficient medicines use, balance resources, and contribute to service improvement while ensuring patient-centred care.

Qualifications

  • BTEC/NVQ Level 3 in pharmaceutical science or equivalent level of experience.
  • Registration with the General Pharmaceutical Council (GPhC) is essential.
  • A full UK driving licence and ability to travel independently are essential.

  • Support the training and development of administrators, PCN teams, and primary care staff.
  • Undertake medicines reviews, audits, and prescribing switches to balance care with NHS resources.
  • Conduct medicines safety audits and review patients per guidance and safety alerts.
  • Support Medicines Optimisation Team in delivering priorities and safety initiatives.
  • Work with GP practices and PCNs to improve medicines optimisation processes.
  • Provide advice on sourcing, procurement, and safe management of medicines.
  • Manage workload and contribute to project planning and service improvement.

Job description

As part of the LMO Team, you will contribute to the development and implementation of the Medicines Optimisation Strategy, helping to meet the pharmaceutical care needs of the local population. Working closely with GP practices, prescribers and multidisciplinary teams, you will support the safe, effective and cost-efficient use of medicines while delivering high-quality patient-centred care.

  • Support the training and development of administrators, PCN teams, and primary care staff to promote safe, effective, and efficient medicines-related processes.
  • Undertake cost-effective medication reviews, audits, and prescribing switches, balancing patient-centred care with NHS resources and budgets.
  • Conduct medicines safety audits and review patients in line with local and national guidance, policies, and safety alerts.
  • Support pharmacists and Lead Medicines Optimisation Technicians in delivering medicines optimisation priorities, including QIPP and medicines safety initiatives.
  • Work with GP practices and Primary Care Networks to improve medicines optimisation processes and support the implementation of medicines incentive schemes.
  • Provide advice on the sourcing, procurement, and safe management of medicines where appropriate.
  • Manage and prioritise own workload effectively to meet agreed objectives and deadlines.
  • Support the planning, delivery, and monitoring of projects and service improvement initiatives.
  • Maintain effective communication and collaboration with a range of healthcare professionals, organisations, and stakeholders.
  • Contribute as a key member of the Medicines Optimisation Team, fostering strong internal and external stakeholder relationships.
  • Utilise and maintain medicines optimisation systems and databases, including prescribing data, cost-saving metrics, and patient outcome information., We know life outside work matters.
