Practice Plus Group's Health in Justice team is seeking a Medicines Optimisation Pharmacist to provide expert clinical advice on medicines and polypharmacy across secure settings. You will deliver medicines optimisation and Structured Medication Reviews, align with national strategy, and lead a team of pharmacists and pharmacy technicians to uphold high standards of care.
