A healthcare provider in England seeks a dedicated Medicines Optimisation Pharmacist. This part-time, fixed-term role focuses on effective medicines management within healthcare settings, emphasizing collaboration with professionals in the field. This is a fantastic opportunity for a dedicated, patient focused pharmacist with excellent communication skills to join our supportive and friendly Medicines Optimisation Team.
You will work alongside pharmacy, nursing and other health care professionals to support and deliver effective medicines optimisation within Locala’s intermediate care and recovery hub settings and will reconcile medicines, undertake clinical medication and falls reviews, provide advice on medicines usage, and deliver education and support to maximise patient’s independence with their medicines.
You will be a registered pharmacist who demonstrates an excellent knowledge and understanding of medicines optimisation, safety, legislation and best practice and be able to apply these in an intermediate care setting. You will have outstanding interpersonal skills and demonstrate the ability to communicate with a range of colleagues and health care professionals.
This is a part time 12 months fixed-term maternity cover post based predominantly at Ings Grove House, but travel may be expected depending on business needs (expenses would be paid) therefore, a full driving licence and access to a vehicle is preferred.
