Job summary

Senior Practitioner Changing Futures, within Oldham Council's Adult Social Care, leads the Enhanced Live Well programme by developing an integrated, multidisciplinary approach to supporting adults experiencing multiple disadvantage.

In this fixed‑term, hybrid role, you will oversee the MEAM Hub as the borough’s MDT, support high‑risk case work, drive trauma‑informed practice, and collaborate with partners to deliver a coordinated, person‑centred response.

Qualifications

• Experience working with adults experiencing multiple disadvantage.
• Strong multi-agency working and leadership skills.
• Ability to manage risk and complex decision-making.

Responsibilities

• Provide operational oversight of complex cases and MDT decision‑making
• Lead and coordinate MEAM Hub activity
• Support staff supervision and practice development
• Work across partners to remove system barriers
• Support data and system evaluation
