Job summary

North Liverpool Primary Care Network is seeking a proactive Care Coordinator to join our dynamic team. You will support patients to access the right services at the right time, working with PCN leaders, practice teams and partners to improve outcomes and patient experience across the network.

Your role involves promoting our services through social media, coordinating outreach work, and contributing to quality improvements.

Qualifications

Responsibilities

With a varied workload and no two days the same, it is ideal for someone who enjoys building relationships and contributing to innovative healthcare services. As a Care Coordinator, you will work with PCN leaders, practice teams and partner organisations to improve patient care and access to services. You will support patients with personalised care needs, helping them navigate health and community services and access the right support at the right time. You will help promote the work of the PCN through social media, patient engagement activities and health campaigns, raising awareness of local services, screening programmes, immunisations and wellbeing initiatives. You will also support the Patient Participation Group, gather patient feedback and contribute to service improvements. Working closely with the PCN Manager, Clinical Director and partner organisations, you will support quality improvement projects, audits and healthcare programmes while developing partnerships that enhance integrated patient care. Undertake work in line with PCN directed priorities. 2. Manager PCN Social Media 3. Proactively identify and work with a cohort of people to support their personalised care requirements, using the available decision support aids 4. Support Link worker manager, with appointment audits and coordinator of outreach work 5. Support PCN Neighbourhood leads 6. Support PCN Clinical director and PCN manager 7. Send Patient surveys for PCN Practices 8. Raise awareness of health promotion in PCN Practices 9. Support PCN Leads with uptake of national screening programmes 10. Support PCN Leads with immunisation programs 11. Support delivery of the PCN covid vaccination programme 12. Direct liaison with multi agencies to coordinate care for patients 13. Lead PCN patient participation group 14. Run PCN Extended access audits 15. To support patient/carer contact roles, and collate patient and carer feedback on their experiences 16. Support Quality and Outcome Frameworks and other DES/LES specifications 17. Maintain and develop PCN engagement with all practice staff and encourage best practice 18. Perform any other duties assigned by management

  • ECDL or equivalent Diploma/ HNC level (or relevant experience)
  • NVQ Level 2 Business Administration (or relevant experience) Demonstrable commitment to professional and personal development, Training in motivational coaching and interviewing or equivalent Oliver McGowan training
  • Excel, microsoft office training
  • Teams training, Experience of working with healthcare professionals and or previous experience in the NHS or social care or relevant field (including unpaid work)
  • Experience of data collection and providing monitoring information to assess the impact of services
  • Experience of partnership/collaborative working and of building relationships across a variety of organisations Working in a multi-disciplinary setting where influence and negotiation is required Knowledge/familiarity with medical terminology, Experience of supporting people, their families and carers in a related role (including unpaid work)
