Job summary

Retain Healthcare is a fast-growing provider of supported living services in and around the Chippenham area. We empower individuals with Autism, Learning Disabilities and/ or Mental Health needs to live fulfilling and independent lives.

As a Support Worker you will deliver personalised support, assist with daily living activities and medication management, and help individuals participate in community life. You will work with a professional, supportive team and receive paid training.

Qualifications

• Experience working with individuals with disabilities.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Patience, empathy, and a positive attitude.
• A valid driver's license and access to a reliable vehicle.
• PMVA qualification or caregiving/social work experience is desirable.

Responsibilities

• Provide personalised support to individuals, ensuring wellbeing and safety.
• Assist with daily living tasks such as personal care, meals, and medication management.
• Support social relationships and community activity participation.
• Promote independence and self-advocacy.
• Maintain accurate records of support provided via Digital Care Planning.
• Collaborate with family, healthcare providers, and social workers.

Key Responsibilities

• Providing personalised support to individuals, ensuring their well-being and safety.
• Assisting with daily living activities such as personal care, meal preparation, and medication management.
• Supporting individuals in developing and maintaining social relationships and participating in community activities.
• Promoting independence and self-advocacy skills.
• Maintaining accurate and up-to-date records of support provided via Digital Care Planning.
• Working collaboratively with other professionals involved in the individual's care, such as family members, healthcare providers, and social workers.

Skills And Qualifications
