Job description

To provide operational day-to-day management and leadership to the major trauma rehabilitation team within the MTC. Work with the trauma coordinators, Rehabilitation Medicine (RM) Consultant, therapy teams, and other medical and nursing staff to support patients discharged home. Liaise with health professionals in other parts of the patient pathway including community therapy teams, GPs, District Nurses, and voluntary and independent sector organisations as needed. Support patients' families and carers.


Coordinate and lead the trauma rehabilitation team multidisciplinary team meetings. Coordinate the rehabilitation care pathway of patients following serious or major trauma admitted to the Major Trauma Rehabilitation ward who need ongoing rehabilitation as a result of their injuries, working as part of the multi-disciplinary trauma team.


Liaise with Therapy, Nursing, and Rehabilitation Medicine staff, facilitating engagement in the rehabilitation prescription process by identifying and coordinating patients for review. Provide specialist advice to support assessment, planning, and implementation of care as part of the team responsible for programmes of rehabilitation for patients who have sustained Major Trauma and serious trauma.


Proactively facilitate transfer to an appropriate rehabilitation setting in a timely manner, where the prescribed programme of care can be delivered. Accountable for own professional actions, not directly supervised.


Have specialist knowledge and expertise in the rehabilitation of patients who have sustained major and serious trauma, including the rehabilitation of patients with musculoskeletal trauma, head injury, spinal cord injury, and other neurological injuries. Provide specialist education and training to other staff and learners.


Ensure data and outcome measures for all patients are recorded and used appropriately to improve patient care and support service development. Carry a clinical caseload of the most complex patient journey through the clinical pathway, providing continuity and advocacy for the patients' rehabilitation needs as they move from one clinical setting to the next.


Work with the trauma coordinators and clinical team to identify patients with ISS over 8, who need a rehabilitation prescription. Have input into the Rehabilitation Prescription process with the clinical teams, using knowledge of the patient and their carers and social circumstances to aid assessment of their rehabilitation needs.


Provide support for patients with ISS >16 and work with the Rehab Medicine Consultant and clinical teams to identify and address rehabilitation needs in this group. Attend and contribute to RM rounds (2-3x weekly) for this group. Proactively support the patient and clinical team in clarifying ongoing rehabilitation arrangements for those moving on to another service.
