Job summary

The role provides patient and family support from diagnosis through treatment, using advanced communication and psychological assessment to alleviate distress and coordinate referrals.

Qualifications

• Registered General Nurse
• First level degree in related subject
• Evidence of continued professional development
• Recognised teaching qualification
• Clinical Diagnostics / Nurse Prescribing
• Evidence of master’s level learning

Responsibilities

• Enable access to information and support patients and families through diagnosis and treatment
• Provide clinical expertise, specialist advice and support across service boundaries
• Define a clear care pathway and refer to other professionals as appropriate
• Make clinical judgements based on experience and knowledge and order tests within competence
• Assess and manage symptoms and know when onward referral is required
• Explain complex information about diagnosis, staging and treatment options
• Support and empower patients and carers to participate in care decisions
• Promote self-management and risk stratification to determine who can self-manage
