Job Title: Primary Care Network Pharmacist

What You’ll Be Doing:
Join our client’s team as a Primary Care Network Pharmacist and play a key role in improving the lives of patients with chronic conditions, with a strong focus on diabetes management. In this role, you will:

Perform comprehensive patient assessments

Create and implement personalised care plans

Deliver evidence-based treatments to optimise patient outcomes

Collaborate with multidisciplinary teams to provide holistic care

Requirements & Qualifications:
To be successful in this role, you will need:

A current and valid GPhC registration

Substantial experience in a Primary Care setting

Strong communication and interpersonal skills

The ability to work independently and within a multidisciplinary team

A commitment to delivering high-quality patient care

Working Hours & Contract Duration:
This role requires availability from Monday to Friday, 9am – 5pm, with potential for future remote work. The contract is ongoing with a strong possibility of extension based on performance and service needs.
