You will contribute to high-quality patient care through clinical reviews, preventative health work and treatment room activity, with well-organised workflows and supportive nursing leadership in place.

Qualifications

• NMC registered nurse with primary care experience.
• Ability to conduct chronic disease reviews.
• Strong patient communication and clinical reasoning.
• Familiarity with EMIS, SystmOne or Vision.

Responsibilities

• Conduct chronic disease reviews for asthma, COPD, diabetes or hypertension.
• Perform wound care, dressings and treatment room duties.
• Carry out immunisations and smear tests (if trained).
• Complete NHS Health Checks and lifestyle assessments.
• Manage results, recalls and routine follow-up tasks.
• Document clearly in the clinical system.
