Responsibilities

• Conduct chronic disease reviews for asthma, COPD, diabetes or hypertension.
• Perform wound care, dressings and treatment room duties.
• Carry out immunisations (if trained) and cytology screening as applicable.
• Undertake NHS Health Checks and lifestyle assessments.
• Manage results, recalls and routine follow-up tasks.
• Document clearly in the clinical system.

You will contribute to high‑quality patient care through a balance of clinical reviews, preventative health work and treatment room activity. The practice offers well‑organised workflows, supportive nursing leadership and a clear structure for daily clinic delivery.

Key Responsibilities

• Conducting chronic disease reviews for patients with asthma, COPD, diabetes or hypertension
• Performing wound care, dressings and treatment room duties
• Carrying out adult and children’s immunisations (if trained)
• Undertaking smear tests (cytology‑trained nurses)
• Completing NHS Health Checks and lifestyle assessments
• Managing results, recalls and routine follow‑up tasks
• Documenting clearly in the clinical system

Requirements

• NMC‑registered Nurse with experience in primary care
• Up‑to‑date immunisation and cytology training desirable
• Confident in chronic disease management and treatment room‑work
• Good patient communication and clinical reasoning
• Familiarity with EMIS, SystmOne or Vision
• Enhanced DBS
