Locum Ready is seeking a proactive Practice Nurse to work with a GP practice in Sevenoaks. You will lead chronic disease reviews, carry out wound care, immunisations where trained, and support routine clinic activities within a structured primary care setting.

The role emphasizes clinical reasoning, clear patient communication and accurate documentation in EMIS/SystmOne/Vision. DBS check updated; part-time regular or ad-hoc sessions available, with long-term bookings offered; NHS Health Checks

Qualifications

• NMC-registered nurse with experience in primary care.
• Up-to-date immunisation and cytology training desirable.
• Confident in chronic disease management and treatment room-work.
• Excellent patient communication and clinical reasoning.
• Familiar with EMIS, SystmOne or Vision.
• Enhanced DBS.

Responsibilities

• Conduct chronic disease reviews for patients with asthma, COPD, diabetes or hypertension.
• Perform wound care, dressings and treatment room duties.
• Carry out adult and children’s immunisations (if trained).
• Undertake smear tests (cytology-trained nurses).
• Complete NHS Health Checks and lifestyle assessments.
• Manage results, recalls and routine follow-up tasks.
• Document clearly in the clinical system.
