Qualifications

• NMC-registered nurse with primary care experience.
• Immunisation training up-to-date desirable.
• Cytology training is a plus.
• Strong clinical reasoning and patient communication.
• Experience with EMIS, SystmOne or Vision.
• Enhanced DBS is required or desirable.

Responsibilities

• Conduct chronic disease reviews for conditions such as asthma, COPD, diabetes or hypertension.
• Perform wound care, dressings and treatment room duties.
• Carry out adult and children’s immunisations if trained.
• Undertake smear tests (cytology-trained nurses).
• Complete NHS Health Checks and lifestyle assessments.
• Manage results, recalls and routine follow-up tasks.
• Document clearly in the clinical system.

This role is ideal for a Practice Nurse with strong assessment skills who can confidently deliver chronic disease reviews, treatment room procedures and routine nursing clinics in a structured primary care environment.

You will contribute to high-quality patient care through a balance of clinical reviews, preventative health work and treatment room activity. The practice offers well-organised workflows, supportive nursing leadership and a clear structure for daily clinic delivery.

Key Responsibilities

• Conducting chronic disease reviews for patients with asthma, COPD, diabetes or hypertension
• Performing wound care, dressings and treatment room duties
• Carrying out adult and children’s immunisations (if trained)
• Undertaking smear tests (cytology-trained nurses)
• Completing NHS Health Checks and lifestyle assessments

Requirements
