Sanctuary Personnel is seeking a Band 6 Pharmacy Technician to join the Medicines Reconciliation Service in Boston. This locum, 3-month contract offers £27 per hour and a full-time schedule, providing opportunities to develop skills in medication safety and clinical governance.

You will conduct medicines reconciliation, collaborate with a multidisciplinary team to optimise patient outcomes, advise on medication regimes, maintain records, and participate in audits and quality improvement

Responsibilities

• Conduct comprehensive medicines reconciliation for patients, ensuring accuracy and safety in their medication therapy.
• Collaborate with a multidisciplinary healthcare team to optimise patient outcomes and facilitate coordinated care.
• Provide expert advice and guidance on medication regimes to both patients and healthcare professionals.
• Maintain meticulous records of all patient medication information, updates, and changes.
• Participate in audits and quality improvement initiatives to continually enhance the service delivered.
