Qualifications

• Experience as a Health Care Assistant or similar role.
• Willingness to work towards a Level 3 Health & Social Care qualification.
• Experience supporting people in a group setting.

Responsibilities

• Support patients and carers during activities.
• Liaise with the multi-disciplinary team for coordinated care.
• Provide administrative support including record-keeping.
