Job summary

Greensleeves Care is seeking a Lifestyle Co-ordinator to plan and deliver engaging activities that support residents’ wellbeing and enjoyment of daily life. You will work with residents, families and staff to tailor lifestyle plans, coordinating weekly programmes and events across our care homes.

Flexibility for evenings or weekends is required. A relevant qualification in health and social care is desirable, and experience in care settings is helpful.

Qualifications

• Experience planning or delivering activities and wellbeing support.
• Understanding of person-centred practice, dignity, safeguarding and confidentiality.
• Ability to work flexibly, including occasional evenings and weekends.
• Desirable: qualification in health and social care or related field.
• Desirable: previous care home experience with older people.

Responsibilities

• Develop, deliver and review individual lifestyle plans for each resident.
• Collaborate with residents, families and colleagues to reflect what matters in care planning.
• Coordinate weekly and longer-term lifestyle programmes including evenings and events.
• Complete risk assessments for activities and outings.
• Build links with relatives, volunteers, schools, faith groups, and community services.
• Manage lifestyle budget and resources with accurate records.
