Job summary

Folkston Park Care and Rehab is hiring a Licensed Practical Nurse to work both short term rehabilitation and long-term care. The LPN supports in developing and implementing individualized Nursing Care Plans, under the direction of the Unit Director, and helps supervise nursing staff.

The role involves assuming leadership responsibilities as needed, attending to daily unit operations per shift, and ensuring high-quality patient care for residents in geriatric and rehabilitative settings.

Qualifications

• Graduate of an approved school for Practical or Vocational Nursing.
• Currently licensed by the State Board of Nursing.
• Experience in geriatric or rehabilitative nursing preferred.

Responsibilities

• Develop and implement individualized Nursing Care Plans with supervision.
• Assist in orientation and supervision of nursing personnel.
• Attend to daily operations of the unit per shift and assume leadership responsibilities as needed.

Position Summary: In conjunction with the Unit Director, the Licensed Practical Nurse utilizes a general understanding of the principles of Nursing in the development of and implementation of individualized Nursing Care Plans to ensure that the needs of the customers are met. He/she assists in the orientation of and supervision of nursing personnel, attends to the daily operations of the unit on per shift, unit level, and assumes responsibilities of a leadership role as needed.

Requirements

1. The Licensed Practical Nurse must be a graduate of an approved school for Practical or Vocational Nursing and must be currently licensed by the State Board of Nursing.
2. Experience is preferred in geriatric or rehabilitative Nursing
