It is an exciting time to join our team as we have recently been commissioned to operate a 24/7, 365-day service. This offers a unique opportunity for individuals with a professional background in Nursing, Occupational Therapy, or Social Work who are interested in helping create a responsive and holistic mental health service. You must be fully qualified in the relevant profession and registered with the appropriate professional body.

We pride ourselves on offering extensive development opportunities, and you will receive strong support from the service to achieve your professional goals.

You will be based across Queen's Hospital, Burton, and the two community hospitals in Tamworth and Lichfield. Some travel across various locations and community settings will be required, and the ability to travel within a geographical area to meet the role's requirements is essential.

Shifts will cover a 24/7 rota. We support flexible working arrangements, which can be discussed during the interview process.

Main duties of the job

The primary purpose of this role is to provide mental health assessments, risk assessments, and treatment, focusing on high-quality assessment and treatment of patients presenting with mental health issues within physical healthcare settings.

You will be expected to:

  1. Provide indirect patient care through formal and informal consultancy, supporting training for acute Trust staff regarding mental health and wellbeing.
  2. Offer specialist assessment and care planning in collaboration with service users and carers, including evaluation of treatment plans and discharge planning.
  3. Facilitate the timely and safe discharge of patients and assist hospital staff in managing mental health risks in the acute setting.
  4. Develop formulations based on individual care needs; implement and evaluate care programs; communicate plans and updates effectively to patients, families, and clinical colleagues.
  5. Conduct thorough risk assessments and promote positive risk-taking to enhance quality of life and independence.
  6. Deliver interventions aligned with care plans and pathway guidance.
