Job summary

Pertemps is seeking a Personal Advisor to join the Leaving Care Service in Newham. You will manage a caseload of care experienced young people aged 18–25 and support their transition to independent adulthood.

You will co-produce Pathway Plans, create risk assessments and safety plans, and provide guidance across health, housing, finance, education, employment and training. The role requires experience with care leavers and a commitment to reflective practice.

Qualifications

• Experience with care leavers or children in care.
• Proven caseload management and co-producing Pathway Plans.
• Ability to assess and manage risk and develop safety plans.
• Understanding challenges faced by care-experienced young people.
• Insight into barriers to EET opportunities.
• Experience in a diverse urban community.
• Commitment to reflective practice and continuous learning.

Responsibilities

• Build trusting relationships with care leavers aged 18–25.
• Co produce Pathway Plans that are realistic, progressive and tailored.
• Create and maintain risk assessments and safety plans.
• Provide guidance across health, housing, finance, education, employment and training.
• Support development of safe family and kinship connections.
• Collaborate with internal and external partners for holistic support.
