You will help learners access opportunities, make informed choices and develop meaningful lifestyles. Key duties include reviewing care plans, safeguarding responsibilities, record-keeping and collaboration with families and professionals.

Qualifications

• Ability to review and update care plans and risk assessments.
• Adherence to safeguarding procedures and reporting concerns.
• Support person-centred learning and encourage community access.
• Maintain accurate records and liaise with families and professionals.
• Contribute to support plans, tutorials and team collaboration.
