Job Description:

Responsibilities include, but are not limited to:

1. Providing face-to-face holistic and comprehensive strength-based care management services to all clients.
2. Liaising and consulting with users, carers, colleagues, and various statutory and voluntary agencies to compile information to assess clients' needs.
3. Ensuring users have access to translation and interpretation services.
4. Facilitating client independence and empowerment as far as possible by ensuring all stages of a client's assessment are strength-based.
5. Producing detailed assessment/care plans as necessary and maintaining up-to-date computerized client records to ensure good social work practice.
6. Participating in regular supervisions with a Senior Practitioner.
7. Providing cover for the work of other team members in their absence as required.

Requirements:

1. Social Work Degree.
2. Registration with Social Work England.
3. Experience of working as a Social Worker in a similar role.
4. Ability to work well independently and as part of a team.
5. Enhanced DBS, dated within the last 12 months, on the update service. You could be fast-tracked through our recruitment process today.
