Job Description:

Responsibilities include, but are not limited to:

  • Conducting comprehensive assessments of individuals with learning disabilities to identify their strengths, needs, and preferences.
  • Collaborating with individuals, families, and relevant stakeholders to develop person-centred support plans.
  • Advocating for the rights and interests of individuals with learning disabilities within the community and various service settings.
  • Providing emotional support and practical assistance to individuals and their families.
  • Collaborating with multidisciplinary teams, including healthcare professionals, educators, and community organisations, to ensure coordinated and holistic support.
  • Facilitating access to appropriate services, including healthcare, housing, education, and employment opportunities.
  • Responding promptly to crisis situations and provide immediate support to individuals and families in distress.
  • Developing crisis intervention plans and collaborate with relevant agencies to ensure ongoing safety and well-being.
  • Maintaining accurate and up-to-date records of assessments, support plans, and interventions.

Requirements:

  • Degree in Social Work or a related field.
  • Registration with the Social Care Wales.
  • Experience working with individuals with learning disabilities.
  • Strong communication and interpersonal skills.
  • Knowledge of relevant legislation and policies related to learning disabilities.
  • Enhanced DBS dated within the last 12 months. On the update service? You could be fast tracked through our recruitment process today.
  • Willingness to complete an enhanced DBS if you do not currently hold one.
