Job summary

Wokingham Borough Council is seeking a Social Care Practitioner to join the Learning Disability Team on a part-time, permanent basis. You will support adults to live independently, stay safe and live well in their communities, with supervision from a social worker or occupational therapist.

Qualifications

• Role requires empathy, respect, and effective communication with diverse groups.
• Good IT skills and ability to learn adult social care recording systems.

Responsibilities

• Carry out strengths-based assessments and reviews of people’s day-to-day lives and support arrangements.
• Gather information from individuals, carers and professionals and keep accurate records.
• Write reports and build positive relationships with residents.
• Work closely with colleagues as part of a team.
