Job summary

Lincolnshire County Council is seeking committed Learning Disability Nurses and Social Workers as Qualified Practitioners to join the Learning Disability South Team, based at Sleaford/Spalding. Two vacancies exist: 1 full-time 37 hours and 1 part-time 18.5 hours per week; salary is dependent on experience.

The role focuses on supporting adults with learning disabilities to live independent lives, completing assessments, developing support plans, and providing long-term person-centred care across

Qualifications

• Qualified Practitioner with Nursing or Social Work registration.
• Experience with adults with learning disabilities is desirable
• Travel to client homes/locations required

Responsibilities

• Complete Adult Needs Assessments and Care Act assessments.
• Develop and review person-centred support plans
• Manage a varied caseload across South Lincolnshire
• Supervise up to 4 staff and support learning within the team
• Work with families, carers and partner agencies to promote independence and wellbeing
