Job summary

Norfolk Primary Care C.I.C. seeks a Non-Prescribing Practice Nurse to deliver high-quality, patient-centred nursing care across both practice and community settings.

This role requires autonomous clinical workload management, comprehensive assessments, and delivering evidence-based interventions within your scope of practice, including care to patients at home. You will work with the Home Visiting Team and multidisciplinary colleagues to support housebound, vulnerable and complex patients,

Qualifications

• Full UK driving licence.
• Access to a vehicle insured for business use.
• Ability and willingness to travel independently between practice sites, patient homes, care homes, and community settings.
• Regular home visiting is a core and essential component of this role and forms part of the routine workload.

Responsibilities

• Provide high-quality, patient-centred nursing care across practice and community settings.
• Undertake comprehensive assessments and deliver evidence-based interventions within your scope of practice.
• Support housebound, vulnerable and complex patients through autonomous clinical care.
• Collaborate with the Home Visiting Team and multidisciplinary colleagues to deliver coordinated care closer to home.
