Job summary

NHS Jobs is seeking a General Practice Nurse to assess, plan, implement and evaluate patient care in a primary care setting. You will work with a multidisciplinary team to promote health, manage long-term conditions, and support self-care for patients.

The role involves vaccinations, family planning support, and adherence counselling in line with national guidelines and local policies. A registered nurse with NHS experience is preferred and the post is on-site in a GP practice.

Qualifications

• Registered nurse with current NMC registration and practising certificate.
• Experience in primary care or general practice settings preferred.
• Knowledge of NHS policies, guidelines and vaccination programmes.

Responsibilities

• Assess, plan, develop, implement and evaluate patient treatment plans.
• Promote health and well-being and support chronic disease management.
• Provide information on medications and safety with adherence support.
• Work with the team to meet QoF targets and infection control standards.
• Lead on patient education, vaccinations, and family planning guidance.
