Job summary

The Leeds Teaching Hospitals NHS Trust is seeking a Specialist Clinical Pharmacist to join the Older Peoples Medicine Pharmacy Team at St James's University Hospital. You will optimise medicines for frail older patients, support polypharmacy management, and work with the MDT to improve outcomes.

You will contribute to education and training of junior pharmacists and may lead service development, audits, and research activities as part of a forward-looking department that values patient-centered

Qualifications

• Registration with the GPhC is required.
• MPharm degree or equivalent is essential.
• Postgraduate Diploma in Clinical Pharmacy is preferred.
• Experience delivering clinical pharmacy services in hospital settings.

Responsibilities

• Optimise medicines for frail older patients and manage polypharmacy.
• Collaborate with multidisciplinary teams to improve patient outcomes.
• Provide expert pharmaceutical care to older adults with complex health needs.
• Contribute to education and training of trainee pharmacists and staff.
