Job summary

Swanton Care & Community Ltd is hiring a Scheme Manager to lead a brand-new supported living service in Colne. This full-time role (40 hours) involves guiding a team to deliver high-quality, person-centred support and enabling independence for people in their own homes.

The ideal candidate has care management experience, NVQ Level 5 in Health & Social Care (or progress towards), excellent communication, and strong leadership.

Qualifications

• Previous care management experience, ideally within a supported living environment.
• NVQ Level 5 in Health & Social Care (or working towards) or an equivalent qualification.
• Excellent interpersonal and communication skills, with the ability to create an inclusive and welcoming environment.
• Proven leadership with proactive management of people, quality and service development.

Responsibilities

• Supporting the Registered Manager with day-to-day operation of the service, ensuring high standards of care and support.
• Leading, supporting and motivating staff to deliver person-centred support that empowers people to achieve their goals.
• Ensuring support plans, risk assessments and care records are accurate and up to date.
• Promoting health, wellbeing and inclusion, meeting diverse needs.
• Supporting with staff supervision, coaching, performance management and development.
• Assisting with rota management, recruitment, inductions and service compliance.
• Building positive relationships with families, professionals and external stakeholders.
• Supporting individuals to access social, educational, leisure and community activities.
• Monitoring quality and compliance and implementing improvements.
• Ensuring records and reports are completed accurately per company policies.
