Job summary

Birmingham Children's Trust is seeking a Lead Practitioner - Targeted Family Help to lead needs-led Family Assessments and multi‑agency plans. You will work with families, ensuring a whole-family approach and act as the main contact for the duration of support.

You will facilitate Early Help and Child in Need meetings, gather information using the social care IT system, and promote safeguarding, equality and inclusion while following Working Together and the National Framework.

Qualifications

• Experience in assessing family needs and working in partnership to achieve excellent outcomes.
• Excellent communication and interpersonal skills.
• Ability to deliver evidence‑based interventions and make effective changes in the lives of children and families.

Responsibilities

• Act as the lead professional for children and their families in need of help and support.
• Lead needs-led Family Assessments and create multi‑agency Family Help plans for families receiving targeted Early Help and/or under section 17 of the Children Act 1989.
• Lead on delivery of multi‑agency targeted Early Help and/or Child in Need plans with evidence‑based help and support.
• Facilitate multi‑agency Early Help and Child in Need meetings to implement and review Family Help plans.
• Promote safeguarding and ensure children’s views are heard in assessments and plans.
