Job description

We are excited to advertise for a Lead Practitioner in the Hospital Discharge Admission Avoidance Team based in Guildhall Shrewsbury. The successful candidate will lead a small team of Social Care Practitioners and Social Workers, focusing on hospital avoidance, integrated community service, adult social care, and wellbeing.

Key responsibilities include operational management, multi-agency collaboration, service improvement, and caseload management. We are looking for someone with experience in leadership, mentoring, and working under the Care Act (2014), with a strong understanding of relevant legislation and guidance related to social care and hospital discharge processes.
