Job summary
Shropshire Community Health NHS Trust is seeking a Lead Pharmacist Prescriber to manage the provision of comprehensive clinical pharmacy services within the Virtual Ward. You will be embedded in the multidisciplinary team caring for respiratory, cardiology and frailty patients and will help advance clinical and pharmaceutical pathways.
The role requires extensive clinical pharmacy knowledge, strong communication and negotiation skills, and experience supervising staff.
Qualifications
- Masters Degree in Pharmacy.
- Registered with GPhC.
- Clinical qualification at postgraduate diploma level or significant relevant clinical experience.
- Registered as independent prescriber.
Responsibilities
- Lead provision and development of clinical pharmacy services across Shropshire, Telford and Wrekin (STW).
- Attend Medicine Governance meetings and support Medicines Management Team to deliver strategic objectives.
- Be a key MDT member for virtual ward.
- Support development of clinical/pharmaceutical pathways for virtual ward.
- Supervise staff and provide education and training.
