Responsibilities

• Conduct comprehensive assessments and create personalised rehabilitation programmes for adults with neurological conditions.
• Deliver high-quality, evidence-based care and regularly monitor progress.
• Collaborate with multidisciplinary teams, residents, and families to promote independence.
• Provide education, advice and support to residents and carers.
• Maintain accurate clinical documentation and adhere to policies.
• Participate in team meetings, supervision, and service development initiatives.
