Job summary

Essex County Council is seeking an experienced Housing Occupational Therapist to support major adaptations and enable safe, timely discharges from hospital and care settings. You will work across Essex with Environmental Health and Housing Officers, focusing on adults and children with complex needs.

The post involves mentoring Health and Community OTs, providing training, and developing pathways with Local Government partners to reduce hospital stays and enable suitable home environments.

Qualifications

• Qualified Occupational Therapist or equivalent qualification with HCPC registration.
• Significant experience in a social care setting with knowledge of Care Act, MHA, MCA and related housing regs.
• Proven track record of delivering major adaptations.
• Experience mentoring staff and being a subject matter expert in professional practice.
• Experience chairing multi-agency meetings with clients, families and professionals.
• Ability to build strong relationships with partners and communities.
• Understanding of financial implications of care packages and commissioning.
• Evidence of continuing professional development.

Responsibilities

• Carry out holistic assessments considering housing factors affecting individuals and households.
• Collaborate with Health colleagues to process referrals for safe, early discharge.
• Refer to universal services and liaise with Housing and other teams to meet needs.
• Provide equipment, support and signposting to meet impairments and housing needs.
• Make recommendations on property types, adaptations and equipment.
• Progress and support recommendations requiring specialist input.
• Monitor referral pathways and provide data for project cost savings.
• Support Disabled Facilities Grant-funded recommendations.
• Ensure referrals are reviewed and progressed to complete adaptations efficiently.
• Support COPs in multi-disciplinary problem-solving with adults, children and families.
• Mentor OTs involved in complex adaptations and provide training on DFG processes.
