Job summary

Northern Care Alliance NHS Foundation Trust is recruiting for a Heart Failure Specialist Nurse/ACP to lead the community heart failure team and work with the enhanced respiratory service. The post will provide autonomous clinical decisions, manage a significant caseload, and support the virtual ward and hospital-at-home initiatives.

You will ensure evidence-based practice, maintain professional ACP standards, and contribute to teaching across nursing staff, trainees, primary care and university

Qualifications

• Advanced practice in cardiology.
• Non-medical prescribing certificate.

Responsibilities

• Lead and line manage the community heart failure team.
• Provide advanced clinical triage of new referrals, assessment and treatment of patients with heart failure.
• Determine clinical diagnosis and treatments and maintain records as an autonomous practitioner.
• Support virtual ward and hospital at home from a heart failure perspective.
• Lead in teaching and supporting junior doctors, nurses, and ACP trainees.
• Develop and review policies/guidelines related to the Heart Failure service.
• Align service with NICE/ESC/GMSCG/GMMMG guidance.
