Chelsea and Westminster Hospital NHS Foundation Trust is seeking an enthusiastic Heart Failure Specialist Nurse to join our integrated, multidisciplinary cardiology team. You will manage your own caseload, run nurse-led clinics, virtual clinics and telephone consultations across both Chelsea & Westminster and West Middlesex sites.

You will provide expert assessment, planning and education for patients with acute and chronic heart failure, support discharge planning and contribute to service

Formación

• Extensive experience in cardiology and heart failure care.
• Ability to work autonomously and manage own caseload.
• Experience with nurse-led clinics, virtual clinics and telephone consultations.

Responsabilidades

• Manage caseload of patients with acute and chronic heart failure.
• Undertake nurse-led heart failure clinics and follow-up arrangements.
• Support education for cardiac inpatients and contribute to service development.
