Job summary

Qualifications

Responsibilities

• Triaging and assessing patients with MDT.
• Develop care plans with community/primary and acute MDT.
• Advanced clinical assessments and treatment development.
• Support Annex 21 pathway with competency development.
• Manage workload and prioritize effectively.
