Job summary

Hywel Dda University Health Board is seeking a permanent Lead Enhanced Clinical Practitioner to join the community team at Eastgate, Llanelli. The post involves advanced assessment and treatment across urgent and community settings, working closely with the MDT to optimise patient outcomes.

Qualifications

• Registered practitioner with current professional registration
• Masters-level CPD and advanced clinical practice training
• Evidence of autonomous decision-making and clinical governance involvement

Responsibilities

• Lead and triage patients in urgent/community settings with MDT
• Develop care plans in collaboration with primary, community and acute teams
• Demonstrate advanced clinical assessment and treatment skills and contribute to service improvements
