Nottinghamshire County Council’s Gedling Family Support and Safeguarding Team is seeking a dedicated Targeted Early Help Case Manager. You will be at the heart of supporting families, assessing needs, developing tailored plans and ensuring positive outcomes for children and young people in the community.

You will lead Early Help cases as the lead professional, coordinate multi-agency meetings, maintain records and collaborate with professionals to deliver effective support in home and community

Qualifikationen

• NVQ level 4 or above in social care, child development or related field.
• Minimum of two years’ experience in Children’s Social Care or Early Help Services.
• Strong understanding of relevant legislation and guidance affecting the service area.
• Proficiency in using databases and word processing software.

Aufgaben

• Lead the way as the lead professional in Early Help cases, following service guidance.
• Assess and plan: complete structured assessments and develop intervention plans for children, young people and families.
• Chair multi-agency Team Around the Family/Child meetings for coordinated action.
• Document diligently: maintain accurate and timely records, data entry and case notes.
• Communicate clearly: work with other professionals to achieve best outcomes for families.
• Engage with families: work directly with families in their homes and other settings.
