Job summary

Bradford District Care NHS Foundation Trust is seeking a District Nurse Caseload Manager for the Unplanned Care team in Bradford. The role combines clinical leadership with caseload management to deliver high-quality home-based care for patients with acute and long-term conditions.

You will lead a team of district nurses and support workers, maintain clinical standards, support development, and help prevent unnecessary hospital admissions. The position is permanent with full/part-time options.

Qualifications

• Leadership and management experience in a clinical team.
• Experience supervising and mentoring staff.
• Strong clinical governance and patient safety focus.

Responsibilities

• The District Nurse Caseload Manager role is predominantly clinical (80%) with 20% non-clinical duties.
• Lead day-to-day caseload management, ensure timely assessment and delivery of care, involve patients and carers.
