Job summary

Oxford Health NHS Foundation Trust is seeking a Team Leader for the North East District Nursing Team to provide clinical leadership, manage a skilled community workforce, and deliver patient-centred care in diverse settings including patients' homes.

The role emphasises reducing hospital admissions, safe caseload management, and driving service development in line with the 6Cs of nursing. Applicants should have a nursing degree and be an experienced leader with strong partnership skills.

Qualifications

• Degree level educated
• Registered General Nurse Adult (RGN) with current registration.

Responsibilities

• Provide strong clinical leadership and management of the team.
• Accountable for the management, development, and supervision of identified staff within the community team.
• Overall accountability for the named district nursing team.
• Clinical holistic assessment and treatment of complex patients.
• Work in partnership to reduce hospital admissions and facilitate discharge.
• Promote independence and wellbeing for patients.
