Lewisham and Greenwich NHS Trust is seeking a Principal Clinical Pharmacist Prescriber to lead the Lewisham Integrated Neighbourhood Team (INT) for Long Term Conditions. You will shape clinical delivery across four neighbourhoods, provide strategic support, and ensure effective team functioning within Lewisham.

Formación

• Degree in Pharmacy.
• Postgraduate Diploma in Clinical Pharmacy or General Pharmacy Practice (or equivalent).
• Registered with the GPC.
• Registered and Experienced pharmacist independent prescriber.

Responsabilidades

• Provide clinical leadership and management for the Lewisham INT for Long Term Conditions.
• Develop, integrate and evaluate the new model of care across 4 Neighbourhoods / 7 PCNs.
• Deliver patient-facing multi-LTC reviews and optimise medicines management.
