Lewisham and Greenwich NHS Trust seeks a motivated pharmacist to work across the interface and community setting within the LIMOS service. You will visit patients at home and in hospital, providing specialist advice and comprehensive medication reviews for older people with polypharmacy.

The role focuses on integrated medicines management, multidisciplinary collaboration, and development of safe prescribing practices across health and social care interfaces.

Formación

• B. Pharm / M. Pharm / BSc (Pharmacy) and GPhC registration.
• Postgraduate Certificate in General Pharmacy Practice (or equivalent).
• Able to practice as a pharmacist within NHS settings.

Responsabilidades

• Provide medicine management reviews for complex polypharmacy and adherence issues.
• Deliver medicines optimisation across interface, community and care home settings.
• Lead on clinical audits and contribute to quality improvement.
• Support safe systems for repeat prescribing and reduce waste in primary care.
• Provide education and training to patients, carers and MDT members.
• Promote cost-effective prescribing and evidence-based practice.
